We, the undersigned, are the parents/guardians

We hereby grant permission for our child to engage in canoeing/kayaking/tubing at TURKEY RUN CANOE RENTAL, INC.

We recognize the risks and hazards of canoeing/kayaking/tubing and hereby release TURKEY RUN CANOE RENTAL, INC. and its owners/operators from all liability and responsibility in regard to any incident or accident which might occur, and hereby agree to indemnify and hold them harmless from any legal action which may result.

We further accept responsibility for return of all rented articles, in as good condition as when received. We still further agree that we will be responsible for reasonably Court costs and Attorney fees incurred by TURKEY RUN CANOE RENTAL, INC. in enforcing this agreement.
